چکیده

EARLY RETROPANCREATIC DISSECTION DURING PANCREATICO-DUODENECTOMY TECHNICAL NOTES (ABSTRACT): Background: Pancreaticoduodenectomy (PD) is the procedure of choice for malignant tumors of the pancreatic head and periampullary region. During PD, early pancreatic neck division may be inadequate, especially in cases of hepatic artery (HA) anatomic variants, when invasion of the superior mesenteric artery (SMA) is suspected, or in cases of intraductal papillary mucinous neoplasms (IPMN). Methods: We perform an early approach of the retroportal lamina (RPL) from behind the pancreatic head, before pancreatic transection. This is accomplished after lymphadenectomy and adequate mobilisation of the specimen from the vessels, on either the neck or the body, according to the tumor extension. Results: We successfully used this approach during 28 PD. There were 21 patients to whom we detected anatomic variants of right hepatic artery (RHA), which was spared in 19 cases; arterial reconstruction was required in one case. We also used this technique in 5 patients with IPMN 3 PD extented to the body and 2 total pancreatectomies, and in other 2 patients with adenocarcinoma involving the porto-mesenteric jonction, requiring limited venous resection and reconstruction. Conclusions: Retropancreatic approach with early RPL dissection is an useful technical variant of pancreaticoduodenectomy which we recommend in selective indications.

Early retropancreatic lamina dissection during pancreaticoduodenectomy: how, when and why?

UNLABELLED Pancreaticoduodenectomy is the procedure of choice for tumors of the pancreatic head and periampullary region. During pancreaticoduodenectomy, early neck division may be inadequate in case of hepatic artery anatomic variants, suspected involvement of the superior mesenteric vessels or intraductal papillary mucinous neoplasms. MATERIAL AND METHODS We describe our early approach to t...

Hind Right Approach Pancreaticoduodenectomy: From Skill to Indications

Background. Pancreaticoduodenectomy is the potentially curative treatment for malignant and several benign conditions of the pancreatic head and periampullary region. While performing pancreaticoduodenectomy, early neck division may be impossible or inadequate in case of hepatic artery anatomic variants, suspected involvement of the superior mesenteric vessels, intraductal papillary mucinous ne...

Role of the Total Mesopancreas Excision in the Surgical Radical Treatment of Pancreatic Head Cancer

In the last five years many authors, both Europeans, and Asiatic have emphasized the role of extended lymph node dissection and resection of retropancreatic tissue as the most significant technical options to achieve a radical intent in Duodenopancreatectomy for cancer. The scientific literature has thus given much importance to the removal of mesopancreas that, according to many authors, is a ...

Posterior approach pancreaticoduodenectomy: does it really improve long-term survival in pancreatic head cancer?

We enjoyed reading the report by Kurosaki et al. on their experience with the left posterior approach to the superior mesenteric artery in a pancreaticoduodenectomy for pancreatic head cancer [1]. Since the retroportal lamina (containing lymphatic structures, small vessels and nerves) should be completely resected during a pancreaticoduodenectomy for pancreatic head cancer, dissection along the...

Using Absorbable Gelatin Sponge to Facilitate Sinus Membrane Elevation during Open Sinus Lift: Technical Notes and Case Series

Ridge atrophy accompanied by the pneumatization of maxillary sinus in the maxillary posterior region may lead to inadequate bone height, thereby precluding implant placement. Therefore, it may be mandatory to perform a sinus membrane elevation procedure and augmentation in the bone. The present study aimed to introduce a novel modification method for sinus floor elevation using a gelatin sponge...
